Submit your completed Activity Report Form via email or fax at 867-456-6502.

To be eligible, the Activity Report Form must be received by March 9, 2012.

Your application will be reviewed and considered by a small group representing Health Promotion, Department of Education, Teachers and RPAY.

The Award will be based on:

• Targeting on healthy beverages and healthy eating
  (ex. Is there a specific issue at your school?)
• Efforts to collaborate and build partnerships
  (ex. Parents, community members, health personnel)
• Uniqueness and innovation of your project
• Incorporating an educational component
